AUTODESK APPROVED MTP INSTRUCTOR AUTHORIZATION FORM
INSTRUCTIONS FOR COMPLETION OF APPROVED INSTRUCTOR GLOBAL AUTHORIZATION FORM
This completed Approved MTP Instructor Authorization Form (“Authorization Form”) is part of the Autodesk Membership Training Provider (MTP) Enrollment process. An Instructor seeking authorization for the first time, or an instructor requesting additional product authorization-to-train, must complete the Authorization Form. Instructor must be affiliated with an MTP before submitting an Authorization Form. Prior to making the authorization request, the Instructor must review the MTP Program Guide (“Program Guides”), as applicable, to confirm they meet the Approved Instructor requirements.
The Instructor will sign and return this completed Authorization Form to the affiliated MTP Organization. The affiliated MTP, and the Instructor, will maintain a copy of the completed Authorization Form to allow for future updates.

- Copy of Autodesk ACU or ACP Certification certificates earned, or any other relevant certificates earned (optional)
